Blepharoplasty, or eyelid surgery, is a technique that aims to treat the effects of aging and is the 3rd most common type of plastic surgery.

In the upper area of the periocular region, several problems could occur:

Generalized lack of volume with eyelid hollowing
Excess volume in the inner corner of the upper eyelid
Excess skin
Prolapsed lacrimal gland
Palpebral ptosis
Marked wrinkles on the glabella
Lack of hair on the eyebrow

Advanced Upper Lid Blepharoplasty

Detailed description

  • In most cases, we remove only the excess skin, correcting the small volume increase present in the inner corner. Drooping or ptosis of the lacrimal gland (where tears are produced) on the outside is rare, but an asymmetry in the eyelid opening occurs much more frequently - one covers the iris more than the other, or both droop.

  • Crowding in this area is very common and adding volume from the upper part of the eyelid to its junction with the eyebrow is crucial to this procedure.

  • Through the same incision for the upper lid blepharoplasty, we can (now with sedation only) also fix the eyebrow in a higher position internally or decrease the strength of the corrugator muscle (which produces the undesirable wrinkles between the eyebrows) that is usually treated with botulinum toxin, thus reducing the wrinkles in this region with a more lasting effect.

Lower Lid Blepharoplasty

Lower Lid Blepharoplasty

Two techniques, subciliary (incision under the eyelashes) or transconjunctival (on the inside of the eyelid without the need for stitches), can be used for a lower lid blepharoplasty. The objective is to reduce and, in some cases, to replace the excess fat in the "crease" area, which appears just below the lid, thus solving two problems at once. This procedure is often accompanied by peels or laser as well as micro- or nanofat grafting.

It is important to emphasize that this is a complex and delicate procedure which should only be performed by plastic surgeons who have sufficient experience in this type of surgery in order to avoid changes in the expression of the eyes.

The eyebrow area is relatively simpler, and it is characterized by the loss of volume, which makes the eyebrow less convex, causing the brow to droop, and by naturally diminishing hair.

Brow lift

Brow lift

Detailed description

Brow lift surgeries consist of simple techniques for removing a small amount of upper skin, performed under local anesthesia, or through endoscopic lifting which treats the area from the inside out under local anesthesia with sedation. Lipofilling for volume restoration is a very common option, similar to procedures performed in a global facial rejuvenation. We very often add volume to the upper part of the eyelid at its junction with the eyebrow.

Signs of aging

  • Fat pockets

  • Scleral show – white area of the eye below the iris becomes visible

  • Marked wrinkles on the skin

  • Canthal tilt – outer corner of eye lower than inner corner

  • Visible lacrimal sulcus

  • Dark circles
